When presented with the question, “How is Aerith’s outfit?”, you’ll have three choices:

  • It’s alright.
  • Looks comfortable.
  • Does it matter what I think?

The “it’s alright” option is the standard Cloud trying to be a nice guy response. Aerith knows this and she takes kindly to your attempt at a compliment. Madam M responds by telling you to take it outside, and this option results in Aerith becoming more fond of Cloud. It also increases the odds of getting the Chocobo Sam side quests.

The “looks comfortable” answer is the neutral response Cloud could give in this situation. Aerith responds to it, but it has no effect on any other part of the story and will not lean you in the Madam M or Chocobo Sam quest direction.

The answer, “does it matter what I think?” is Cloud’s attempt at excusing himself from the situation. Aerith perceives this as rude and will not like this at all. You should only choose this option if you have no desire to be close with Aerith, and it improves your odds at the Madame M side quests.
