You might have a tough time leveling this weapon to the max, but it becomes easier as you unlock some of the attachments we show here. Here’s the best Milano 821 loadout in Black Ops Cold War:

- Barrel: 10.1″ Reinforced Heavy
- Body: Steady Aim Laser
- Underbarrel: Field Agent Grip
- Magazine: 45 Rd Drum
- Handle: Field Tape

Starting things off with the optics, we are using Millstop Reflex. This scope offers good visibility and enough magnification to make it ideal for a submachine gun.
The Muzzle will be the Agency Suppressor, as it provides some flash concealment and a good boost to vertical recoil control.
For the Barrel, we are using the 10.1″ Reinforced Heavy. It’s a toss-up between this one and the 10.6″ Task Force. Either works well on the Milano, so take your pick. The bullet velocity and damage range increases are necessary for this SMG because the damage drop off is terrible at mid-range and greater.
On the Body, we are putting on the Steady Aim Laser. The laser is a must-have for any SMG because you’ll sometimes be using hip-fire in close-range situations.
The Underbarrel will have the Field Agent Grip, which is essential for fixing the Milano’s horizontal and vertical recoil. It adds 6% to the vertical and 20% to the horizontal, turning this weapon into a laser beam.
The Magazine we will be using is the 45 Rnd Drum, but you can also go with the Vandal Speed Loader if you want to get some quicker reload action. Either is fine, and this gun does not have the highest fire-rate, so you can experiment with a few options.
On the Handle, we will be using the Field Tape attachment. It provides a considerable boost to flinch resistance, meaning you can stay on target even if you’re taking fire.
Lastly, we will be using the Wire Stock for the Sprint to Fire Time boost. You’ll be doing a lot of sprinting and sliding around corners, so this should help out more often than not.
