The Cosmic Wheel tarot card can only be summoned by Fortuna at a specific point in the game. After Aedana’s corpse visits Fortuna and reveals to her the true nature of her powers, Fortuna has an existential chat with Abramar.
Abramar jokes that she should draw a card to see what she should do about it, and you’re given the option to either agree and actually draw one, or disagree and not take the risk. It’s heavily implied that something very bad could happen if you decide to draw a card, so I honestly don’t blame you if you decided to play it safe instead.
If you do draw a card, it will be because Fortuna has decided to command her deck to obey her. By making this demand and subsequently drawing a card for the answer, she’ll draw a card with The Cosmic Wheel’s likeness on it, rather than a card from her own deck.
Once The Cosmic Wheel has been summoned and Abramar is done drooling over your powerful capabilities as a witch, it will be added to your deck; once it’s in your deck, you’ll be able to randomly draw it during readings like any other tarot card.
