How to get to the Secret Cow Level in Diablo 2 Resurrected
To get to the Secret Cow Level in Diablo 2 Resurrected:
- Complete the game on the difficulty in which you want to open the Secret Cow Level (Normal, Nightmare, or Hell).
- Obtain Wirt’s Leg from Tristram.
- Obtain one Tome of Town Portal from Akara.
- Transmute Wirt’s Leg and the Tome of Town Portal in the Horadric Cube.
- Enter the Secret Cow Level.
That’s how you open the Secret Cow Level in Diablo 2 Resurrected. There are some slight changes in this version in comparison to vanilla Diablo 2. Killing the Cow King now no longer locks you out of the Secret Cow Level, and that means you can now fully clear the level and not have to worry about not being able to rerun it.
In the olden days, when dinosaurs still roamed the earth, if you locked yourself out of the Secret Cow Level, you would need to rely on other players to open the portal. It was a minor inconvenience, to say the least.
How to get Wirt’s Leg in Diablo 2 Resurrected
To get Wirt’s Leg, you will need to interact with the Cairne Stones in the Stone Field and enter Tristram. Find and interact with Wirt’s Body, which is on the far northwest corner of Tristram. After interacting with the body, Wirt’s Leg will pop out. Loot the item, and teleport back to town to throw it in your stash or proceed with opening the Secret Cow Level.

The Secret Cow Level is a hidden area that’s populated with nothing but cow enemies called Hell Bovines. There are also elite and unique packs or mobs to defeat and the big bad boss Cow King. It’s a great area to farming levels quickly due to the high-density level of enemies. Many players also like running the Secret Cow Level to obtain Crystal and Broad Swords for Larzuk in Act V to four-socket.
