When asked to drink from one of the two cups in Chapter 2 of Mother route, choose the right cup. Choosing the left cup will get you killed as it is poisoned and it will be game over for you.

This situation is preceded by your conversation with the Slaver where you need to play “Death’s Table” if you want to get information from him. You will notice that even upon selecting the left cup there will be some blood on it signaling that something is wrong with that one.
